The Department of Otorhinolaryngology and Anatomy at SMVMCH organized an intensive and highly practical “Hands-on Cadaveric Endoscopic Sinus Surgery Workshop” on 13th April 2025 at the Anatomy Dissection Hall, College Block. Running from 9:00 AM to 4:00 PM, this full-day workshop aimed to equip postgraduate students with real-time anatomical orientation and operative skills essential for mastering Functional Endoscopic Sinus Surgery (FESS).

The event attracted enthusiastic participation from postgraduates across multiple medical colleges, offering them a unique hands-on opportunity to work on cadaveric specimens under expert guidance.

Workshop Highlights:

This workshop served as a practical extension of the previously held CME and quiz sessions, focusing on enhancing technical proficiency, spatial understanding, and endoscopic maneuverability in sinonasal surgery.

Orientation & Surgical Anatomy Briefing
Hands-on Cadaveric Endoscopic Sinus Surgery Workshop

The day began with an introductory session covering endoscopic anatomy of the nose and paranasal sinuses. Faculty provided a brief review of

• Key anatomical landmarks: middle turbinate, uncinate process, ethmoid bulla, and sphenoid sinus • Endoscopic views and navigational tips • Variations in anatomy that influence surgical strategy

Instrumentation & Endoscope Handling

Participants were introduced to FESS-specific instrumentation including:

  • Rigid nasal endoscopes (0°, 30°)
  • Blakesley forceps, suction elevators, debriders, and microinstruments Emphasis was placed on proper hand positioningsafe instrument introduction, and ergonomic use of the endoscope, which are critical to minimizing surgical errors.
Hands-on Cadaveric Dissection
The core component of the workshop involved cadaveric dissection under supervision. Each participant performed:
  • Uncinectomy
  • Middle meatal antrostomy
  • Anterior and posterior ethmoidectomy
  • Sphenoidotomy
  • Frontal sinus exploration
  • Orbital nerve decompression
  • Endoscopic exposure of the lacrimal sac
  • Trans Maxillary exposure of Pterygomaxillary Fissure and identification of Maxillary artery were done by the candidates who showed the necessary surgical inclination
Under faculty supervision, participants were guided through identifying surgical landmarks, understanding tissue planes, and executing precise dissection techniques. The step-by-step practical exposure reinforced anatomical knowledge and built surgical confidence.
Doubt Clearing & Feedback Session

Conclusion:

The Hands-on Cadaveric Endoscopic Sinus Surgery Workshop was a resounding success, offering postgraduate students an invaluable opportunity to translate theoretical knowledge into real surgical skills. The program emphasized safe surgical practices, anatomical precision, and hands-on experience, which are vital for any budding otorhinolaryngology.

The Department of Otorhinolaryngology extends its sincere thanks to the Department of Anatomy for logistical support and cadaveric resources, the faculty for expert mentorship, and the participants for their dedication and enthusiasm.

The Department of Otorhinolaryngology, SMVMCH, successfully conducted a focused and interactive CME on “Hands-on Cadaveric Endoscopic Sinus Surgery” on 12th April 2025 at the SMVMCH Auditorium. The session aimed to provide postgraduate students a thorough understanding of endoscopic sinus surgery through anatomical dissection, imaging interpretation, and expert surgical demonstrations.

This CME brought together resource faculty from various specialties — Anatomy, Radiology and Otorhinolaryngology — to deliver a multi-disciplinary approach toward mastering Functional Endoscopic Sinus Surgery (FESS).

1. Anatomy of Nose and Paranasal Sinuses

Resource Faculty: Dr. Priyadarshini N.A., Professor, Department of Anatomy, SMVMCH

The program began with a foundational lecture on the detailed anatomy of the nose and paranasal sinuses (PNS). Dr. Priyadarshini emphasized clinical anatomy relevant to endoscopic sinus surgery, including the osteomeatal complex, turbinates, ethmoid air cells, and critical anatomical landmarks like the lamina papyracea and skull base.

The session provided essential knowledge needed to avoid complications during sinus surgery and highlighted anatomical variations that influence surgical approaches. Her session was well appreciated for integrating cadaveric visuals and 3D anatomical correlations.

2. How to Read CT – PNS

Resource Faculty: Dr. Joe Vimal Raj, Assistant Professor, Department of Radiology, SMVMCH

In the second session, Dr. Joe Vimal Raj offered an insightful talk on interpreting CT scans of the PNS, focusing on preoperative imaging assessment.

He explained how to systematically evaluate a CT-PNS (axial, coronal, and sagittal views) and identify anatomical variants such as agger nasi cells, Haller cells, Onodi cells, and septal deviations. He also emphasized red flag signs on imaging, such as proximity to the orbit and skull base erosions, and demonstrated how radiological findings guide safe surgical planning.

Resource Faculty: Dr. Sivaraman Ganesan, Professor, JIPMER

The session walked participants through the step-by-step surgical procedure, including uncinectomy, middle meatal antrostomy, ethmoidectomy, and sphenoidotomy. Dr. Ganesan elaborated on surgical technique, instrument handling, endoscopic orientation, and anatomical preservation during FESS.

This session offered an invaluable surgical perspective to budding ENT surgeons and was praised for its clarity and practical approach.

4. Complications of FESS and Their Management

Resource Faculty: Dr. Mary Kurien, Ex. Professor, PIMS

In the concluding session, Dr. Mary Kurien discussed potential complications of FESS — including bleeding, orbital injuries, cerebrospinal fluid (CSF) leaks, infection, and adhesions.

She stressed the importance of preoperative planning, intraoperative vigilance, and timely recognition of complications. Management strategies for each complication were explained in detail, backed by real case examples from her extensive experience in sinus surgeries.

Her talk served as a crucial reminder of surgical safety, patient monitoring, and post-operative care, closing the CME on a note of caution and clinical prudence.

Conclusion:

The CME on Hands-on Cadaveric Endoscopic Sinus Surgery provided postgraduate students with theoretical knowledge, radiological insights, and surgical experience. The emphasis on surgical anatomy, imaging interpretation, and a live procedural overview made it a highly impactful and educational session for young ENT aspirants.

The Department of Otorhinolaryngology and Anatomy express their heartfelt thanks to all the expert speakers, participants, and support staff who contributed to the success of this well-curated academic event.

The workshop concluded with an interactive feedback and Q&A session, where participants discussed challenges they faced during dissection and clarified doubts with faculty. Constructive feedback was shared, and participants were encouraged to integrate this learning into future clinical practice.
